Enrollment Process
Becoming a student at Sunnnydale Adventist Academy involves completing the appropriate paperwork and getting the necessary legal, financial and medical clearances. This page outlines the procedure needed to enroll at Sunnydale the forms needed. These forms are in PDF format and require the Adobe Reader to view and print them.
1. APPLY FOR ADMISSION
A. Complete the Application/Student contract form. The information supplied on you Application Form becomes a part of your permanent record so please fill it out completely.
B. Arrange to have three (3) Recommendation forms filled out and mailed to SAA by people who know you well, such as your pastor, your boss, a teacher, or an adult friend.
C. If a 9th grader, send a copy of your diploma. If a transfer student arrange to have a copy of your transcript sent to SAA. If acceptance is granted without a diploma or transcript being submitted, it is conditional until they are received.
D. Send items A and C to Sunnydale along with a refundable $50.00 room deposit if applying as a dorm student. This step completes the application process. Upon receiving a letter of acceptance, complete steps 2, 3, and 4 below.

2. OBTAIN FINANCIAL CLEARANCE
Fill out the financial paperwork (Financial Plan (Dorm), Financial Plan (Village), Federal W-4, State W-4, I-9, Work Agreement and Employment Application) and make satisfactory arrangements with the Business Office. Please take care of this before August 1, 2010. Financial clearance is not granted until a financial plan has been completed, approved, and the first payment is received.
3. MEDICAL RECORDS
A. Consent for Medical Treatment form must be signed by the student’s parent/guardian.
B. According to Missouri State Law, immunization records must be on file in order for students to attend classes.
C. Physical examinations are required of all new students and student planning to participate in sports activities before school begins. Your may use the physical exam form available here.
4. ATTEND REGISTRATION
Plan to complete the admission process by registering for classes on Sunday, August 15, 2010. Bring your parent or guardian, as there will be legal forms for them to sign.
